Appointments & Office Hours:
All office visits are by appointment only. Our staff is
available by phone at 610.789.0150 Monday through Friday, 9am to 5pm EST to
assist you in making an appointment. We are unable to make appointments online
at this time. We try to accommodate everyone but Dr. DiNubile's patient office
hours are sometimes filled weeks or months in advance. If you are unable
to keep your appointment, we ask the courtesy of calling us to cancel or
reschedule so we can accommodate other patients in need. Please take a moment to
review our policies and some important tips for both new and existing patients.
We encourage you to complete your necessary
patient-related forms before your visit.

Cancellation Policy:
Please call us 24 hours in advance if you must cancel or
reschedule your appointment. Calling us allows us to accommodate other patients
in need and they really appreciate it. This is especially true for New Patients
or Existing Patients with new problems since extra time is allotted for you. All
changes to appointments must be done by phone.
Billing & Insurance Information:
We accept most major insurances including Medicare,
Independence Blue Cross, Personal Choice, Keystone Healthplan East, Aetna,
Cigna, AmeriHealth, United Healthcare, Keystone Mercy, Worker's Compensation,
and Motor Vehicle Accident (MVA). Please contact the office at 610.789.0150 to
check if we participate in your particular plan. You will need to bring your
insurance card or other health coverage information to your visit. If we are not
in your plan then full payment is expected and appreciated at the time of
service. Many plans require a co-pay on your part and the co-pay is expected at
the time of service.
There are many new "rules" for both the physician and
patient that are dictated by the insurance carriers that can, at times, be a
source of inconvenience and frustration. We don't make these rules but must
follow them. If you are enrolled in an HMO, you will need a referral from
your primary care physician before being seen. Most HMO's do not allow us to
take X-Rays in our office, so you may need to obtain them before your visit,
thru your primary care physician. If you have questions regarding any bill or
invoice received after your office visit please contact our
billing service, PMA (Practice Management Advisors) at 800.322.4606 and ask for

What to bring to your office visit:
- Current insurance information and cards
- Co-pay and deductible payments
- Any insurance required referrals
- A list of all medications you are taking
- Walking or workout shorts (knee examination only)
- Tank top or short sleeve shirt (shoulder examination
only)
- Any pertinent X-rays, MRI's, medical records, operative
reports (op report), lab studies or office notes (see areas
below for New Patients and/or Existing Patients for details
on this very important matter)
New Patients:
Welcome to our practice! There are some things you can do
that will make us more effective in helping you get better and allowing us to
better answer all your questions. Please fill out your
patient-related forms.
If you have not had any prior evaluation, studies or treatment for your
condition then see the above "what to bring to your office visit" section.
If you have had any prior evaluation or treatment for your condition, we need
you to do some homework to get us your information.
Please bring pertinent X-rays, MRI and lab work (if done). With X-rays and
MRI, please do not have the films sent or mailed to us (they never seem to
arrive). Rather, you will need to go to the facility where they were taken and
pick them up. Please check that the correct films are in the folder and that
they are actually your films (mistakes do happen and this will save you a second
trip). Also it is very important for you to have the reports of your X-rays and
MRI sent to us. They can be FAX'd to us at 610.449.9814 or you can bring the
reports, along with the films, to your visit. If the studies are over 5 years
old, then they are less likely to help us and you don't need to get them. If you
have had prior surgery then we will need your op report which can be obtained
from your surgeon or sometimes from the hospital or surgi-center where your
surgery took place. Records from prior treating physicians are also helpful.
The more information we have, the better position we are in to help you. Also

visit section" and if there are any additional questions, contact us.
Existing Patients:
Welcome back! You are probably familiar with our policies,
but please take a moment to review things. If you have not completed our
...forms please do so. If there has been a change in your address, phone number,
contact information, health insurance or billing information please let us know
and bring your new cards and information to your visit. Also update any
significant changes in your health (ie. new allergies, medications, surgery, or
medical problems). If we have not seen you for several years, please remind our
staff prior to your visit and we will obtain your old chart and records from our
storage area.
If you have been evaluated or treated elsewhere for your
problem, we will need you to do some homework. Please bring pertinent X-rays,

Questions and Emergencies:
We are always happy to answer your questions
and concerns. We believe that an informed patient is a happier patient who is
more likely to do well. If you have medical questions, problems or special needs
you are encouraged to call the office at 610.789.0150 during regular hours. If
you have an emergency after hours, our regular office phone number will connect
you with the answering service. The answering service will contact the doctor on
call, who will respond to your emergencies. Please do not call after regular
hours to make, reschedule or cancel an appointment or to discuss prescriptions,
insurance, or non-urgent questions. Calls after regular business hours should be
on an emergency basis only.
